Athletics Permission Form - 2026-2027

Hello Jefferson parents, 

We are excited to have our Blue Jays return back to school on Monday! 
If your child will be participating in any sports this school year, please fill out the attached Athletic Permission form before the first day of practice on Monday, August 17th. **Student-athletes will not be able to participate without this form being completed. 

Practice times for sports will be 3:00-4:00 on Monday, Tuesday, and Thursdays with gamedays on Fridays. 

It is important to note that this form is being sent to 3rd-6th grade parents. 3rd graders will only have participation ability in Spirit cheer. 4th graders will be able to participate in Cheer, Cross Country, Wrestling, and Track. 5th and 6th graders will be able to participate in all sports that we offer here at Jefferson.  

Fall sports (Football, Girls Volleyball, and Cross Country) will start on August 17th.
